LLETZ procedure | Glengarry Private, Perth

A LLETZ is where abnormal cells found on a smear are removed with a heated loop.

 

Why do I need a LLETZ?

To remove abnormal cells that are too severe to go away on their own. It’s important to remove them with a LLETZ before they develop into cervical cancer.

 

How long does the LLETZ take

It takes about 10 minutes, and is usually very straightforward.

 

How is a LLETZ performed?

It’s similar to a coloposcopy.

  1. You lie on a couch.
  2. A speculum visualises the cervix.
  3. A colposcope – a special kind of microscope – is used to see the abnormal cells.
  4. Local anaesthetic is used.
  5. The abnormal cells are removed and the area sealed with heat. This doesn’t hurt.
  6. The sample is examined in the lab to check all are removed.

 

After the LLETZ

  • You might get some cramps. Take paracetamol or ibuprofen.
  • You’ll get a blood-stained discharge for a few days.
  • The discharge becomes watery for a few weeks.
  • Use a pad and avoid sex and baths while the discharge is there.

 

Risks

  • Infection (5%)
  • Not all cells removed (3%)
  • Reaction to anaesthetic (<1%)
  • Premature labour in pregnancy. This varies with the size of the biopsy.

 

You need follow up if any of the following occurs after your LLETZ

  • Strong smelling discharge
  • Heavy bleeding is common in the second week, but usually settles after a few hours.
  • Severe abdominal cramps

 

Avoid

  • Moderate or strenuous exercise for 2 weeks.
  • Tampons for about 4 weeks.
  • Sex for about 4 weeks.
  • Baths and swimming for about 4 weeks

 

Results

  • Results are available in 1-2 weeks.

 

Follow up

  • A follow-up smear & colposcopy is nearly always required after 4 months.
  • This is to make absolutely sure all cells have been removed at the LLETZ.
  • You will then need a smear plus HPV screen after 6 months then yearly until you’ve had two negatives. Then back to normal screening.

 

Stop smoking if you smoke

Stopping smoking enhances your body’s ability to fight the pre-cancerous cells.
